There was a fourth possibility. The power supply was too weak to handle the graphics card. It has been fixed. The link has been added.
Did you PERCHANCE check the connectivity? Take a multimeter and see if the slots match each other. Mind you, you need to have the PC and GPU active.
Other than that, get a new power supply. GPUs aren't cost-effective when it comes to power usage, they are power hungry. I had to throw out a 300W PSU and buy a 500/600W Corsair PSU specifically for my nVidia GPU.
